| Was in a rush at my 28 week app so forgot to ask for it. Work know am still waiting to get one and seem fine with it but i finish for mat leave in 2 weeks.
Does it have to come from a mw or can i get from my gp? And if mw, is a community team so apps hard to come by, so should i go up to maternity ward at hosp and get done there or do you think they would see me locally just to sign and give me the form if i went at the beginning or end of their appointment day?

| I would call the community midwives office, they may do one for you just to go in and pick up. I think it has to come from the mw not your gp x |

| Maybe its dif here but I got mine from my GP. He filled it out wrong and I asked MW for a new one the next week and my MWs cant issue them anymore. Even if your MWs do issue them I see no reason why your GP couldnt also? When I went to ask for it to be amended I just brought it to receptionist desk and she took my details and I collected the correct one the next day- didnt even physically see a GP. |

| I phoned my GP office and asked to speak to the MW (on the day I knew she would be in) and asked if she could fill out my MATB1 form and I picked it up from the GP reception on my way home from work.
Just give your GP a call, say to the receptionist your at work and cant get it until such and such and you just need your MATB1 form |

| My work wanted docs stamp on it too, so might be worth getting that on while you there.
Plus photocopy it if anything like my work they lost it...
